AVTS 3.13.1.4 challenge voter info

Colgan. Challenge voter cards on occasion fail to record the challenge
voter information (name, address, etc), but are still recorded as a challenge
ballot. This causes the review challenge ballots dialog to internal
error. Work around for the internal error will be in the next release, as
well as a check so that the bogus cards are caught when they are
read.
