Gems 1.15.9 - "Include On" Header Option

The option "Include On with checkboxes Accu-Vote or Accu-Vote T" does not appear to be working accordingly.  Created 2 headers, one for the avts with the Accu-Vote T selected and one for accuvote with Accu-Vote selected.  The header appears correctly on the avts, but the ballot creates both headers.
